Hoosier Care Connect

HoosierCareConnect

Hoosier Care Connect is a health care program for individuals who are not eligible for Medicare and are aged 65 years and older, blind, or disabled who reside in the community and receive SSI.

 

With Hoosier Care Connect, individuals select a health plan that takes care of their physical, behavioral, medical and social needs, coordinating their care with their medical provider(s).

 

Benefits include dental and vision services, pharmacy and prescription drugs, care coordination services, assistive devices, referrals, preapprovals and additional benefits.

 

To apply, individuals must meet these eligibility factors:

  • Blind or have a disability

  • Age 65 and older

  • Living in the community

  • Not enrolled in Medicare

  • Getting Supplemental Security Income (SSI)

  • Enrolled through M.E.D. Works

  • Children who are wards of the state, receiving adoption assistance, current and former foster children
